• Your Responses Are Strictly Anonymous - No ID information is collected or recorded.
• You can respond to the survey once.
• If you return to a partially-completed survey you can continue where you left off and can edit your original answers.
  
• For each award category, please submit the Name and Callsign of your nominee.
• If you have more than one nominee for a category, please number them in order of preference:
  (1) = K6XYZ, (2) = N6ABC, (3) = W6QLF ,etc.
• Keep in mind, that only PAID members of NCCC are eligible for these awards. Check the club ROSTER to see if your Dues are up-to-date.
• For reference, the Names & Callsigns of previous winners are shown for each category.
• Repeat winners are allowed EXCEPT for the Rookie-Of-The-Year. 
  

  The winner of each category will be announced at the annual NCCC Awards Meeting (Date/Time TBD).
